What to Bring to your First Podiatry Appointment
- Your insurance card
- Your driver’s license or a form of photo I.D.
- List of current medications, including dosage and schedule
- Records from previous doctors about your current foot condition

All co-pays, deductibles and co-insurance amounts are due at the time of service. We accept Cash, Check, Visa & MasterCard.
